Request Transcript

1. Applications and Paying-in Vouchers for Certificates/Transcripts are available at the Dean’s Office, Faculty of Dental Sciences.
2. If you wish to download the application form and paying-in voucher from the Dental Faculty web site, the completed application form should be sent along with the bank deposit slip.
   Ensure to enter the Student Registration No. and the Name of the Applicant as reference when depositing. (Please keep photocopies of all the mailed documents and bank deposit slip.)
3. Transcripts are sent to Universities, Ministries, Embassies or other recognized Institutes only. Official transcripts are not issued for the student/applicant.  The Name and Address of the Institute the transcript needed to be sent has to be stated clearly (preferably in block capitals) in the application.
