Submission process

a) How to apply for ethics clearance?
Complete the application form and submit it with the relevant documents to the email; erc_newproposals@dental.pdn.ac.lk

b) What needs to be submitted?
1. Duly completed application form signed by the Principal Investigator.

3. Research proposal (word document and the PDF without the names of the investigators)

4. Information sheets, consent forms, assent forms ( where necessary in all three languages) – Word/ PDF documents (The soft copies of sinhala and tamil translations must be submitted as PDF documents).

5. Another set of Information sheets, consent forms, assent forms ( where necessary in all three languages) without the names of the investigators to be forwarded to the reviewers (PDF/word as appropriate)

6. Questionnaire forms ( where necessary in all three languages).

7. Data collection forms if applicable.

8. A short, uptodate curriculum vitae of each investigator (including members of the ERC or its subcommittees)
- Incomplete applications or applications submitted without the required documents will not be accepted.
- The ERC meeting will take place every third Tuesday of the month, except in the months of April and December. Only applications submitted at least 2 weeks prior to the next ERC meeting shall be included in the next ERC agenda.
